  • Planning details (to whom was your activity directed: Church members? local community? What were the aims?)
  • Activities undertaken: describe this in some depth – we need to get a real flavour of the work done:
    • Why did you decide on this activity?
    • What happened?
    • Where did it take place?
    • When did it happen? A one-off event, a series, drip-drip-drip of information over long period?
    • Who was involved (church members? Outside agencies? Local community groups?) and how many?
    • Any outcomes measured and how
